To enquire about enrolling your child at Crow's Nest State School, please contact our office on (07) 4698 3111.

Enrolling your child at Crow's Nest State School is a simple process. Contact the school to initiate this process and have all enrolment documents mailed to you or download all our information and enrolment documents now.

Parents, please come and visit the school – we will be very happy to guide you through our grounds and certainly this is the best method of getting the feel for our school.

Documents to be signed and/or returned to the school

Enrolment checklist for families (PDF, 90KB)	Checklist of documents to be completed and returned for to school for student enrolment
Application for student enrolment (PDF, 406KB)	To be completed and signed by parents/carers prior to enrolment interview.
Birth Certificate	A copy of the child's birth certificate may be required to be included with your completed Application for student enrolment if requested.
Enrolment agreement (PDF, 160KB)	To be signed during your enrolment interview by parent, student and school administration staff. By signing parents/carers and students agree to the school's conditions of enrolment.
Computer network and internet agreement (PDF, 172KB)	By signing parents/carers and students agree to adhere to the school's network and internet policy.
Media consent form (PDF, 174KB)	To allow the school to use you child's image, name and work in school publications.
Religious Instruction - Parent Information (PDF, 169KB)	Parent information about religious instruction at the school.
Voluntary student participation in program of Chaplaincy services (PDF, 79KB)	Form to allow child to participate in specific activities with religious or spiritual content with School Chaplain.
Athletics Permission Form (PDF, 208KB)	Form to give permission for medium/high risk Track and Field Events
Enrolment Statements (70KB)	Form to give acknowledgement of school policies and permission for minor local excursions
Application for P&C membership form (PDF, 228KB)	Parents/carers interested in joining the P&C should complete this form and return it to the school.
